《保险研究》20250406-《长期护理保险待遇给付政策对个体待遇选择的影响：基于情境实验研究》（陈鹤、王振宁）

［摘   要］完善长期护理保险待遇给付政策有助于实现制度目标、增强制度可持续性。本研究利用情境实验设计面向L市长护险受益人收集一手数据，以二层多分类Logistic回归分析保险待遇给付政策对受益人待遇选择的影响。结果表明：长护险待遇给付政策对待遇选择具有引导作用。在以社平工资作为参照的政策属性中，护理津贴的边际效应最大，护理津贴占社平工资的比例每增加1%，选择该类待遇的受益人比例增加0.50%（P<0.001）；其后是居家护理封顶线（0.17%，P<0.05）、机构护理封顶线（0.10%，P<0.01）的边际效应。在报销比例类政策属性中，机构护理（0.15%，P<0.001）和居家护理报销比例（0.16%，P<0.01）的边际效应比较接近。各项政策属性的影响在不同居住安排、教育、收入群体中存在异质性。鉴于此，本研究建议发挥长护险待遇给付政策对受益人待遇利用行为的引导作用，谨慎采用护理津贴形式的待遇，加强长护险待遇给付政策效果的预判研究。

The Impact of Long-term Care Insurance Benefits Policy on Individuals′ Benefit Choices：A Vignette Experiment Study

Abstract：Enhancing the benefits policy of long-term care insurance （LTCI） is crucial for achieving the program′s objectives and ensuring its sustainability.This study employs a vignette experimental design to collect primary data from City L and utilizes a two-level multinomial logistic regression model to analyze the impact of LTCI benefits policy on individuals′ benefit choices.The results indicate that the benefits policy significantly influences these choices.Among the policy attributes referenced to the social average wage，care allowance had the largest marginal effect （0.50%，P<0.001），followed by home-based care payment cap （0.17%，P<0.05） and institutional care payment cap （0.10%，P<0.01）.Specifically，each 1% increase in the care allowance relative to the social average wage results in a 0.50% rise in the proportion of beneficiaries selecting this benefit.Among the reimbursement rates-related policy attributes，institutional care （0.15%，P<0.001） and home-based care （0.16%，P<0.01） exhibit similar marginal effects.Furthermore，the policy′s effects vary based on living arrangements，education，and income.This study suggests that policymakers should use the benefits policy to guide beneficiaries′ decisions，exercise caution with cash allowance benefits，and thoroughly assess the potential impacts of these policies before adoption.
